Personal Protective Equipment Expense Reimbursement (PPE)
For any additional PPE supplies that businesses need for re-opening, the City of Maple Valley will reimburse eligible businesses up to, however, not exceeding $300.00 per business. Please visit the Association of Washington Business PPE Connect website at www.reboundandrecovery.org/ppe-connect to locate a supplier that can assist you with your PPE needs. Note: If you purchase items directly from a supplier, please submit your receipt to the City for reimbursement. If a business doesn’t have the funds available upfront to purchase the PPE supplies needed, they can submit a copy of their invoice (with the price quote) so that funds can be issued to allow the business to place their PPE order. Please submit any receipts or invoices for reimbursement to Finance2@maplevalleywa.gov Note: Your W-9 Form (Taxpayer Identification Number and Certification) and Maple Valley Business License Number (UBI number) need to be submitted along with your PPE receipts. If your organization is exempt, a UBI number is not required. Please include an Invoice Number that is generated by your organization when submitting your documents for reimbursement.
